Don't know about the Tokays,,,though I would guess both a dish and a mist would help. In regard to dusting, I dust daily or every other day for babies and juvies and every 5-7 days for adults.

After a little research,,,seems Tokays prefer to drink dropletts off of leaves, etc. So best guess would be to mist a few times a day.

Mealworms and waxworms can help fatten him up, but don't let him get to used to them. They should be a treat for a healthy gecko, not a staple.

you could use feeding tongs...I dont know if he will take the mealies off them but its worth a try! If not I guess just put them in a dish that they cant get out of...thats all the advice from me I know nothing about tokays except that they can be mean lol

Re. water - he will more likely drink water droplets off leaves and the sides of the cage so make sure you give him regular mistings, however, I also like to keep water bowls in all of our enclosures so that the animal can drink as and when it wants to. Just make sure the water bowl isn't too deep that he could drown or not get out, though he probably won't spend much time near it.
You can try feeding the worms with the use of tweezers - I find when you have a grip on them they move around a lot more but some lizards won't take from tweezers, in which case if you leave them in a bowl with high sides so they can't get out your gecko should take them from there if he wants to. They do move a fair bit and he will be able to see them.
Add calcium and vitamin powder/supplement to the live food every other feed, and wax worms are high in fat, but as Ken said don't let him get too hooked on them as then it can be hard to get him to take other things.
